Placement Process
When we receive your call, we will begin the Needs Assessment form which will allow us to determine the level of care needed. After this initial phone consultation, Nancy will begin a search of the area resources, and then meet with you in person (unless the situation is an emergency) to discuss your best options.
At that point, Nancy will coordinate the schedule of on-site visits to the most appropriate care homes, or meetings with in-home care providers. If needed, Nancy will meet again with the individual &/or family to determine the need to explore other resources. These may include:
- realtors who specialize in senior clients,
- moving / organizing assistance agencies to assist with the move &/or an estate sale,
- financial advisors to explore untapped benefits
- Veterans benefits advisors
Once the individual has moved into a care facility (or the in-home care services have begun), Nancy will make a personal follow-up visit to ensure their needs are being met and the transition is going well. Throughout the year, we visit our contracted facilities to determine that the quality of care continues to meet the State guidelines, as well as our own referral standards.
